Intake
We will come to your home to meet you and your family. A nurse will develop a care plan based on your skilled home healthcare needs.

Start of care
A scheduler from our office will coordinate with you and your therapists to start your care as soon as possible.
Referral & Assessment
Call us and we will help you get started. Then your physician contacts Seaside Home Health with a referral. We are going to start creating a customized care plan for you. Our clinicians design a plan tailored to your needs, goals, and lifestyle.
Care at Home: Skilled nurses, therapists, and caregivers bring expertise and compassion into your home while you recover.
Ongoing Support: We stay in touch, adjusting care as your needs evolve, always ensuring comfort and safety.
